HEALTH SERVICES INFORMATION

Immunizations Mandatory
for Students
Tennessee
state law requires that all children entering Williamson
County Schools must have proof of all immunizations
completed. Any student who does not meet these state
requirements will be suspended from school until proof is
received. If your child is entering our Pre K program,
there are different requirements. Please see your school
nurse.

VACCINE |
DOSES |
COMMENT |
| DTP/DTaP/DT/Td |
4 or 5 |
A
fifth dose is not required if the fourth dose is
given after the child's fourth birthday. Only
three doses are needed if DT or Td must be used and
the first dose is given after 12 months of age. |
| Polio |
3 or 4 |
If
the third dose is given after age four, the fourth
dose is not required. If the child has
received both OPV and IPV, a total of four doses are
needed regardless of age. |
|
Hepatitis B |
2 or 3 |
Required for kindergarten entry (three doses) and
seventh grade. For adolescents, a two dose
vaccine preparation is available. |
| MMR |
2 |
Required on or after the first birthday; first dose
may be given no earlier than four days before the
first birthday. |
|
Varicella (Chickenpox) |
1 |
For
kindergarten entry only. Required on or after
the first birthday; may be given no earlier than
four days before the first birthday. Parental
or physician diagnosis of chickenpox also meets the
requirement. |
In order to
enroll a new student into Williamson County Schools,
parents/guardians must bring a Tennessee Immunization Record
signed by a physician or by the Williamson County Health
Department for their child to the school in which they are
enrolling to begin this process. So if they are moving from
out of state, this Tennessee Immunization Record must be
obtained from a physician or the Williamson County Health
Department before attempting to enroll at the school itself.
Proof of a physical that has been completed within the past
year must also be received if you are a new student to our
district.
Williamson
County School Health Partnership will continue in the
Fairview and Page school clusters. The partnership between
the Health Department and our school district will continue
to benefit our students for the year 2008-2009. With
written permission from parents, school nurses will be able
to provide several services that would not be available to
students. Services such as physicals, over the
counter medications, and many health, nutrition and fitness
programs are available.
