IMPORTANT INFORMATION FOR BHS ATHLETES

In order to participate in athletics at BHS, all athletes read this document:  2009 WILLIAMSON COUNTY ATHLETIC PHYSICALS.doc.

* * * Athletes who do not have a baseline physical exam on file will not be allowed to practice * * * 

If you are taking advantage of the FREE physical exam at the Bone & Joint Clinic in Franklin on May 20, 2009, at 5:30 p.m., you MUST take the following forms with you to the physical exam:

  1. Bone & Joint Clinic Physical 2009 Form.pdf

  2. Student Athlete Authorization for Disclosure of Protected Health Information

  3. Medication List

If you schedule your sports physical with your personal physician, the following physical exam forms need to be filled out by your doctor and returned to the BHS Athletic Office or the coach of the sport in which you are participating.

  1. TSSAA Physical Form--Emergency Treatment

  2. TSSAA Physical Form--Emergency Treatment

  3. Student Athlete Authorization for Disclosure of Protected Health Information

  4. Medication List
