BMS Sports Physical Information

Physical Forms

If your child is planning to participate in middle school athletics next year, they must have a current physical on file before being allowed to try out.  Baptist Sports Medicine will be at Woodland Middle on Thursday, May 15th to help students meet this requirement.  The cost of the physical is $10.00.  Physicals will begin at 6:00 p.m.

All students need a sports physical form in order to try out for a sport at Brentwood Middle.  Please follow this procedure:

  1. Obtain a sports physical form from the office or the BMS website.  (Please note that there are 2 forms for students in the 7th grade.)

  2. Get a physical from your doctor-preferably after June 1st so that it can used all year.

  3. Make at least 5 copies of the signed physical form.

  4. On the first day of tryouts, the student should give a copy of the physical form to the coach.  Students will need to do this for each sport in which they participate.

  5. Do not give the original copy to the coach!


Rule Change for 7th Grade Sports Physicals

Starting with the 2008-2009 school year, the State Board of Education requires that every athlete in the 7th and 9th grades receive a complete well child checkup before they can participate in sports.

 This new rule prevents upcoming 7th and 9th graders from taking part in the mass physicals offered at some of our schools.  These students will need to go to their primary care provider, nurse practitioner, or a specially trained public nurse or registered nurse to get this comprehensive check up.  A part of this comprehensive check up involves lab work.  When upcoming 7th and 9th graders receive the complete well child checkup please be sure the person completing the physical fills out the new form.  Since this is a new rule medical facilities may not have a copy of the new form.  A copy of this new form may be picked up in the school office.

 All students wishing to participate in athletics during the next school year must have a physical after May 1st to be eligible.  The new rule only applies to upcoming 7th and 9th graders.  Students in other grades will still be able to get the same physical as in years past.
