SPORTS PHYSICALS for $10.00
Date – May 12^th, 2008
Time – 6:00 p.m. to 8:00 p.m.
Where – Fairview Middle
Cost - $10.00 per student
(provided by Baptist Sports Medicine)
Directions:  All students must be accompanied by a legal parent or guardian.  All current 5^th and 7^th graders in the Fairview and Hillsboro area can participate.  Please enter through the gym or the back of FMS like you would during a sporting event.  The front doors will be locked.  To help the flow and limit your wait time, please try to come during the appropriate time for your student:

5^th graders – 6:00 -7:00                              7^th graders – 6:30 – 7:30

Current 6^th and 8^th graders must have a complete physical. These sports physicals will not work for them due to a new state law.

For Rising 9th graders only -  Free Sports Physicals - May 13, 2008 in Franklin

The first two forms linked below, Participation and HIPPA release, will be used for those student-athletes going through STAR’s free physical on May 13^th 5:45 at the Franklin Bone and Joint. On this date all incoming 8^th grade students are welcome. All incoming 6th, 7^th and 9^th grade student-athletes will be required to visit their PCP for a well child care visit. The third form Interscholastic will be used for all 7^th and 9^th grade student-athletes.
