Out of Transportation Zone Request 2009-2010

1.    ALL OUT OF ZONE REQUESTS MUST BE SUBMITTED FOR APPROVAL BY MAY 15th, 2009.

2.     Parent will obtain 2009-2010 out of transportation zone form either by website or at the school.

     3.    Parent will complete the top portion of the form (STEP 1).

4.    Parent will go to the zoned school to obtain the principal’s signature (STEP 2).

5.    Upon completion of STEPS 1 and 2, parent will go to the out of zone school to obtain that principal’s signature (STEP 3).

6.    The out of zone school staff will initial, date and time stamp the request on the top right corner of the form when all three steps are complete.  This will determine the order that students will be considered for processing.

7.    The out of zone school staff will make two copies of the form: one for the parent and one for the school.  The original will be sent to the Central Office for consideration.  Parents must keep a copy of the out of zone form until the process is complete.

8.    Students who make a request from schools other than Grassland Middle, Independence High and Ravenwood High schools to attend Brentwood Middle, Centennial High or Page High schools will not be considered or notified of acceptance until after the May 15th deadline.

9.    When considering out of zone requests from Page High to Centennial High and Centennial High to Page High, the amendment approved by the School Board was that neither Page nor Centennial would lose any staff as a result of students “swapping” schools.  For example, because of current staffing ratios (PHS 18:1) (CHS 21.7:1), if 22 students go from Centennial to Page then at least 18 students would need to go from Page to Centennial.  Out of zone requests submitted prior to May 15th will not be approved if the number of out of zone students causes a staffing change at either Page or Centennial.

10. All other requests will be processed on an ongoing basis.
