  • Rising 7th Grade Immunizations

All students that will be entering 7th grade will be required to bring in a current immunization record showing all vaccine dates since birth, plus the additional 7th grade Tetanus vaccine, before the first day of school. Immunization Records can be turned in to the nurse as soon as this is completed.

If the requirements are not met, students will not be allowed to attend school.

    The requirements are:

    • One dose of TDAP (Tetanus Booster)·
    • It must be on a TN Certificate of Immunization form, which your doctor will have. *The bottom left corner section 3, should be checked “complete 7th grade or higher.” * In section 4, it must show the physician signature and address of the office.
    • Physical exams are required only if entering public school for the first time or for participation of any interscholastic athletic team. We prefer an original copy of the physical exam.
    • Please make your doctor appointments now so your child can start on the first day of school. An invalid immunization certificate with proof of
a doctor’s appointment is not allowed for school entry.
    • Once you have obtained your valid certificate, you will want to make a copy for your records. They will need it for college.
    • Certificates can be brought into the front office, faxed or scanned and emailed. Please do not mail certificates as they could get lost in the mail.

If you have any questions, please feel free to contact Nurse Gloria.
